Functional or locomotor disabilities

Difficulty climbing less than 10 steps of stairs

≥ 2 eligibility criteria

Life expectancy less than 6 months

Difficulty moving or reduced walking speed

A chronic disease affecting participation to the program

Difficulty walking over 400 m without break

MMSE< 20

Walking time less than 1 hour per week

Feeling of exhaustion during domestic activity

>  2 falls during the last year

Involuntary weight loss (> 5% in 1 month or > 10% in 6 months)
