Inclusion criteria

Exclusion criteria

• 65 years or older

• Medical conditions such as the presence of: unstable angina, uncontrolled congestive heart failure, unstable arrhythmia, COPD stage III or IV which contraindicate following a program of physical activity

• Resident in Barcelona, community-dwelling

• Assigned to one of the 8 PHCC

• Can attend on-site the consultation room at the PHCC

• Will stay in the reference area a minimum of one year and a half

• Home Care Program or institutionalization at baseline. Planned admission to nursing home

• Frailty inclusion criteria:

• Participation in other physical activity program

• score of 1 point or above in the Barber Questionnaire

• Has been operated on hip and/or knee the last 6 month (walking independently with technical assistance is not a contraindication)

• Fried modified frailty criteria: 3 or more

• Gait time between 10 to 30 seconds in the Timed Get Up and Go test

• Suffering a non-controlled neoplastic disease, terminal or severe disabling illness

• MEC-35 of Lobo ≥18 points (no severe cognitive impairment)

• Cannot understand Spanish

• Capable of consent. Agreement to participate in the study
