Medication chart model will not entirely solve our problem. Although the medication chart is your legal document you still need medications prescribed for different conditions at different times like urine infection, vomiting and diarrhoea, constipation so you still need someone to add those medications onto that document

You could have situations where you are not sure whether that was the original medication chart that had been submitted for claiming

GPs

I would be in favour of using medication charts but that would require somebody to monitor the pharmacies delivering medications. How many prescriptions (i.e. medications) they dispense in our name because we will then no longer be responsible for the number of scripts going out in our name †
