Data collection flow chart. This flow chart outlines the process that the research nurses at respective sites will use to recruit patients for the study from inpatient hosptial wards and the biopsychosocial data that was collected at baseline. The flow chart further outlines the three types of patient follow-up that will be conducted in order to determine medication harm eight weeks following hospital discharge. This includes review of any hospital re-admissions, a patient interview and review of General Practice records. These three sources of information will finally be triangulated to determine overall if medication harm has occurred
